Placing a parent or loved one in a skilled nursing, long term care, or assisted living facility is a life changing milestone. Understanding the mechanics of these settings is a challenge to patients and their families. They want to know that their healthcare providers, including those providing their primary care and behavioral health services, are focused and dedicated to these settings.

Our Physicians, Nurse Practitioners, Psychologists, and Licensed Clinical Social Workers are all specialists in providing services in skilled nursing, long term care, and assisted living facilities (the post-acute space). Our goal is to provide patients with the highest quality and most efficient level of care, while managing the various driving forces effecting post-acute facilities, including utilization costs, hospitalization and re-hospitalization rates, lengths of stay, and the new Patient Driven Payment Model (PDPM) for skilled nursing reimbursement.

We provide care to our patients using a dynamic team-focused approach with a team of physicians and nurse practitioners dedicated to providing the best possible care to the unique needs of residents of skilled nursing, long term care, and assisted living facilities.

We have developed a proven multi-specialty medical and behavioral health management program to address the hazards, inefficiencies, and over-utilization of resources that exist in today’s fragmented healthcare system, and to eliminate the inefficiencies that drive up the cost of healthcare.

Centric Physicians accomplishes this by providing robust and frequent face-to-face medical management of our patients, by utilizing evidence-based clinical protocols, and by coordinating with patients and families to assure the necessary and appropriate medical care is provided at the right time and in the right location.

Our behavioral health service plays a vital role in our patients’ healthcare. As the age of the geriatric population continues to advance, many patients face adjustment disorders, depression, bi-polar disorder, cognitive decline, and dementias. We provide a full scope of behavioral health services from testing and diagnosis to ongoing treatment and counseling. Effective communication between our primary care and behavioral health teams leads to improved outcomes and a better quality of life for our patients.
